Hospice provides a "special" kind of caring for terminally ill patients, their families and their caregivers that:


Responds to the physical, emotional and spiritual needs of patients and families, and believes the quality
of life to be as important as the length of life.

Takes place in the patient's home or in a home-like setting.

Concentrates on making patients free of pain and comfortable so they can make the most of the time that remains to them.

Considers helping family members an essential part of its mission.


Hospice Admission Criteria

Attending physician has determined the patient is in the last phase of a terminal illness.

The patient/family/caregiver agree that patient care will be provided primarily at home.

The focus of care desired must be palliative versus curative.

Patient must reside within the geographical area in which the services are available.

The patient and family must be aware of the diagnosis and prognosis.

The patient must have a life-threatening illness with life expectancy of six months or less, as determined by the attending physician and Hospice Medical Director.

A responsible caregiver can be available.
