PEIA Weight Management Plan

The Family Fitness Center is the only approved site in the area to offer the PEIA Weight Management Program. This program is designed to help you lose weight and improve your health. As an exclusive provider, the Family Fitness Center will provide participants with a comprehensive weight management plan suited to their unique and individual needs. You will receive the services of a dietitian, an exercise physiologist and personal trainers while gaining access to a fitness facility meeting PEIA’s quality criteria.

Why is PEIA Offering this Benefit?
PEIA began the Weight Management program in the summer of 2003 with one site and has been expanding the program since. Member and claims reports indicate that the participants who stick with the program experience improved health and lower claims costs. The program does supply the necessary supports for those people who are committed to making a permanent lifestyle change.
The program takes discipline, support and a willingness to change your lifestyle for good. Even a modest weight loss of 10% can improve your blood pressure, cholesterol and glucose levels. It can make activities of daily living easier like walking, tying your shoes, carrying groceries, playing with the kids, sleeping well at night, and lessoning joint pain.

Who Qualifies for the Weight Management Program

•You must have the PEIA Preferred Provider Plan as your primary coverage;
•You must have a Body Mass Index (BMI) of 30 or greater or a BMI of 25 or greater with a related medical condition like heart disease, high blood pressure, diabetes or metabolic syndrome;
•You must be ready to make permanent lifestyle changes;
•You must have the approval from your physician or nurse practitioner.
You may be excluded from consideration for the
following reasons:
•You recently underwent bariatric surgery;
•You participate in the Dr. Dean Ornish program for Reversing Heart Disease;
•You are not ready;
•You have previously participated in the program;
•Your physician or nurse practitioner does not approve; •­There is not yet a participating facility in your area

What Services Will I Receive?
Dietitian Services:
An individualized nutritional assessment with follow-up visits

Exercise Physiologist Services:
A fitness assessment with follow-up visits

Personal Trainer Services:
Individualized fitness plan development and implementation
Access to the weight management facilities’ fitness facility so long as you participate in the program

Care Management Services:
Assistance from specially trained Lifestyle Management nurses

How Much Will It Cost?
There is no coinsurance or deductible, but you will have copayments which are as follows:
Cost: $20 per month
Second year: you pay half and PEIA pays half of the facility fee up to a total of no more than $50 per month.

Since the program is a once per lifetime benefit, participants are advised to use it only if they feel ready… participants who drop out will not be able to return to the program.
